Scott Hill is also fortunate to have two Service Coordinators on-site and available to residents five days a week.
What is a Service Coordinator?
A Service Coordinator’s role is to help coordinate the provision of supportive services to assist residents in maintaining their health and independent lifestyle. They manage and provide access to supportive services in the community, provide case management services as needed, and develop programs to promote wellness to the entire resident population.
For example, they design and implement wellness-related services and activities that support residents’ well-being and independence without any charge to the residents. Programs may provide consultation, blood pressure checks, exercise opportunities, and education. Referrals to other specialized professional services (such as physical therapy or home health) may be arranged but will be subject to standard charges of the health service agency when requested by the individual resident. *A meal program is included for
Briarcliffe residents. Having the meal program allows those residents a reduced rate for a monthly meal program. The evening meal is served 365 days per year. Residents in Ridgeview have the option to participate in the meal program.
